My mailing address has changed. If your course materials were shipped before April 1, 2004, you will need to make an adjustment to the mailing label on the self-addressed, stamped envelope that was included with your course materials before mailing back your logbook and triplicate grade report forms.

Please print the mailing label below and glue or tape it over the existing mailing label on the envelope.
